Care Associate Behavioral Health

POSITION SUMMARY

Care Associates provide support to behavioral health clinical teams. Care Associates complete various tasks including customer service, benefit review and documentation, inbound and outbound calls to providers and members. Care Associates work with Behavioral Health Clinicians and other Clinical Support Staff.

Fundamental Components:

- Assists members locate services.
- Facilitates follow-up calls.
- Collects information on patient progress and adherence to treatment plan
- Provides information to members regarding mental health and substance abuse benefits, community treatment resources, mental health managed care programs, and Aetna policies and procedures.
- Reviews patient compliance with treatment plan; identifies obstacles to compliance; works with Care Managers to develop action plans.
- Follows-up with members regarding missed aftercare appointments.
- Follows-up with outpatient providers to assure timely outpatient follow-up appointments for patients discharged from the hospital.
- Serves as liaison between the inpatient staffs of local hospitals and high volume provider groups to assure timely discharge appointments.

BACKGROUND/EXPERIENCE desired:

1+ years of customer service experience is required
Experience navigating multiple computer systems is required

1+ years Behavioral Health experience working with mental health population is preferred

EDUCATION

The highest level of education desired for candidates in this position is a High School diploma, G.E.D. or equivalent experience.

Telework Specifications:

- Full-Time Telework (WAH) for current WAH employees.
- New employees: Telework will be considered. Employees must live within an hour of an Aetna office.
