POSITION SUMMARY
As a Care Manager, you will help support the Amazon In Touch Care Premier Dedicated team to provide comprehensive coordination of medical services for the membership.

Fundamental Components:
This is an in-office Care Manager position in the San Antonio office. This position is in an office environment using a computer station and may require sitting for extended periods of time. Candidates must possess good computer skills. This position is ideal for Registered Nurses seeking a fast paced environment. Care managers are needed with general medical/surgical and/or specialty experience. The care manager works closely with all members of the care team to telephonically assess the member's health status and care coordination needs, review inpatient chart notes in preparation for collaborative and proactive discharge planning, develop and implement a member-centric case plan, monitor and evaluate the case plan, involve the MD as indicated and end the working relationship with when the member has become independent with self managing their healthcare needs. The care manager will also identify opportunities to enhance the members overall wellness through integration with vendor partners.

BACKGROUND/EXPERIENCE required:
  • RN with current, unrestricted TX license & willing to obtain additional licensure
  • 2+ years clinical practice experience in the acute care/hospital setting & managed care experience desirable.
  • Candidate must possess strong customer service skills including: attention to customers, sensitivity to certain issues and proactive identification/resolution of issues.


BACKGROUND/EXPERIENCE preferred:

  • BSN
  • Managed care experience
  • Case management
  • CCM preferred but not required & must be willing to work toward CCM.


EDUCATION
The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.

LICENSES AND CERTIFICATIONS
Nursing/Registered Nurse is required
Nursing/Certified Case Manager (CCM) is preferred
