RN Field Assessor

Primary Location: Princeton, New Jersey
Additional Locations: NJ-Essex County, NJ-Morris County, NJ-Passaic County

This is a full time RN Assessor who will conduct face to face visits to Aetna Members and completes needs assessments for personal care assistance services, Adult and pediatric medical day care services, and MLTSS clinical eligibility.

This position requires nursing clinical judgment, travel, In office training, and work at home computer data entry.

Fundamental Components:

Assessment of Members: Through the use of clinical assessment tools and information/data review, conducts comprehensive assessments of referred member’s needs/eligibility and determines clinical eligibility for requested services and/or meeting needs by evaluating member’s benefit plan and available internal and external programs/services.Applies clinical judgment to the incorporation of strategies designed to reducerisk factors and address complex clinical indicators which impact care planning and resolution of member issues. Assessor must ensure visit timeliness as percontractual requirements and be able to communicate needs in a professional manner to the case management teams. Must have strong clinical documentation skills.

Background Experience:

2 + years clinical practice experience; Required
Knowledge of electronic health documentation and MS office products; Required
RN License in NJ active and unrestricted; Required

Experience in care management, discharge planning, and/or clinical assessments; Preferred
Bilingual Spanish Fluency; Preferred
Residency in Passaic, Morris or Essex Counties; Preferred

Ability to travel to members homes in assigned counties to conduct assessments; Required
Ability to Train in Princeton office for approximately up to 6+ weeks; Required

Additional Job Information:
Position is field based and candidate Must be able to travel to members homes in assigned counties toconduct assessments.
Training in Princeton office for approximately up to 6weeks+ Based on Successful Completion or Training and Leadership Approval.
Typical office working environment with productivity and quality expectations
Work requires the ability to perform close inspection of hand written and computer generated documents as well as a PC monitor.
Sedentary work involving periods of sitting, talking, listening. Work requires sitting for extended periods, talking on the telephone and typing on the computer.
Ability to multitask, prioritize and effectively adapt to a fast paced changing environment
Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding
Effective communication skills, both verbal and written.
Travel to office for training is required. Position after training will requires traveling to complete face-face visits at members home and applicant must have ability to work from home.
