Case Manager (RN)

Primary Location: Phoenix, Arizona
Additional Locations: AZ-Phoenix

This is a telework role with 10-25% of community based travel. An ideal candidate will reside in the West Valley of Phoenix, AZ.
This role has standard business hours, Monday-Friday.
POSITION SUMMARY
This is a licensed Registered Nurse position in the West Phoenix area to care manage dual eligible members with multiple health issues. This includes both Physical and Behavioral conditions. There will be periodic travel with face to face visits conducted to ensure coordination of care for this vulnerable population.

Fundamental components include but are not limited to:
-Coordination of care with providers, community agencies, caregivers etc.
-Working with members to elicit change in maintaining an optimal healthy lifestyle.
-Educating members on plan benefits and being an advocate to them.
-Meeting federal and state regulatory requirements

BACKGROUND/EXPERIENCE
An active and good standing RN license is required
3+ years clinical practice experience required
Experience in integrated care management preferred
Possess excellent critical thinking skills and time management required

EDUCATION
The minimum level of education desired for candidates in this position is a Bachelor's degree or equivalent experience.

Nursing/Registered Nurse (RN) is required
