Telephonic Care Manager - Oncology

Aetna’s Engagement Hub is staffed with a member centric team serving as the front line of customer service for Corporate Care Management. Our team consists of Engagement Specialists, Care Advocates, Telephonic Care Managers, and Social Workers dedicated to serving Aetna members.
Our Oncology Care Manager will demonstrate a highly energetic blend of clinical experience, health care coaching, and care management expertise. The Oncology Care Manager will provide short-term care management support to members of Aetna’s Cancer Program through a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy.


Fundamental Components:
Responsible for short-term telephonic care management,
serving eligible Aetna members contacted through Aetna’s Cancer Program
Assesses the member’s current situation, identifying gaps in
care, and determining resource needs.
Partners with the member to close care gaps and provide one-on-one
assistance managing through the member’s short-term care management needs
Enrolls members who demonstrate a need for long-term care
management support in the applicable Aetna Care Management Program
Utilizes influencing and motivational interviewing skills to
ensure maximum member engagement; serving as the member’s advocate to ensure
they receive the services and support they need
Helps members actively and knowledgeably participate with
providers in healthcare decision-making and accurately identifies
community resources
Reviews the current literature regarding effective engagement and
communication strategies, oncology treatment and patient support best practices,
and care management strategies – incorporating this knowledge into the Aetna
Cancer Program
Demonstrates proficiency managing email in an Outlook account and using remote communication
software such as Skype and WebEx; able to demonstrate proficiency with Word,
Excel, and experience documenting within an electronic health record

Knowledge of oncology conditions, evidence based guidelines, prevention, wellness, health risk assessment, and patient education is strongly preferred
Minimum of 3 years care/case management experience required
Minimum of 3 years clinical experience required
1+ years of community-based experience preferred, particularly within an interdisciplinary care team
Critical thinking skills and ability to analyze complex data sets.

Ability to manage complex care management issues utilizing assessment skills and protocols
Health Plan experience preferred
Bilingual (Spanish) preferred


Education:
Licensure as a Registered Nurse is required; a Bachelor’s degree is preferred
