Clinical Case Manager BH

Primary Location: Houston, Texas
Additional Locations: TX-Houston

Position will be at an offsite location in Houston, TX (northern Houston, near Spring, TX).
Hours can fall between 9am and 7pm, some Saturdays may be required
Flexibility in the role and hours
POSITION SUMMARY
This position is for a licensed social worker or RN licensed in Texas and located in the Houston area. The main focus will be to provide behavioral health case management service to the public from a centralized hub setting. A strong knowledge of the services and recourses available in Houston area is a requirement.

Fundamental Components:
This position will be a part of a very innovative team that is focused on assisting patients to their best health.
Accessing for gaps in care
Supplying services and resources in the area
Assisting patients to navigate complex health care
Providing live group coaching for BH topics, i.e.; depression, stress and anxiety

BACKGROUND/EXPERIENCE:
3+ years of direct clinical practice experience post masters degree, e.g., hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility
Case management and discharge planning experience preferred
Managed care/utilization review experience preferred
Crisis intervention skills preferred
Public speaking preferred
Working with groups preferred

EDUCATION
The minimum level of education desired for candidates in this position is a Master's degree or equivalent experience.

Mental Health/Licensed Clinical Social Worker (LCSW)
Mental Health/Licensed Master Social Worker (LMSW)


FUNCTIONAL EXPERIENCES
Functional - Nursing/Case Management/4+ Years
Functional - Communications/Communication Delivery - Public Speaking/4+ Years


TECHNOLOGY EXPERIENCES
Technical - Desktop Tools/Microsoft Outlook/4+ Years/End User
Technical - Desktop Tools/Microsoft PowerPoint/4+ Years/End User
