Telephonic Social Worker (LSW)

Description:
This role partners with a
registered nurse case manager to utilize a collaborative process of assessment,
planning, and advocacy to meet a member’s benefit plan and/or health needs. Responsibilities
include researching available options and resources for members with financial,
transportation, safety and housing needs. Population spans the entire
lifecycle. All member contact is done telephonically or via electronic
exchange. This position works to empower members to make positive lifestyle and
medical choices and works as a team with other clinical staff to assist members
with community resources for medical, financial and behavioral health.


Fundamental Components:

Utilizes clinical judgment
and critical thinking skills to help member’s achieve maximum physical and
behavioral health through assessment and care planning, direct provider
coordination/collaboration to promote effective utilization of available
resources resulting in optimal, cost effective outcomes. Utilizes motivational
interviewing skills to ensure maximum member engagement. Provides coaching,
information, goal development and support to the member and the case manager assisting in order
to empower the member to make ongoing independent medical and lifestyle
choices.



Background Experience:

2 years experience in medical social work, case management and/or discharge planning.
Complies with all state requirements in the state where job duties are performed.

EDUCATION required

Bachelor's or Master's degree must be in Social Work where required by state law.
Bachelor’s degree in Social Work, Psychology or Gerontology required.
Masters degree preferred.


LICENSE required
Requires an LSW or MSW with unrestricted active license for the state of North Carolina.

Multiple state licensure/certification preferred but not required.



Additional Job Information:
Knowledge of the regulations, standards, and policies which relate to social work practice.
Knowledge of individual and group behavior and inter-relationships among social, economic psychological and physical factors.
Extensive knowledge of community resources.
Thorough knowledge of casework and group work principles, practices, and methodology.
