Care Management Associate

Primary Location: Harrisburg, PA
Additional Locations: PA-Harrisburg
Support comprehensive coordination of medical services including Care Team intake, screening and supporting the implementation of care plans to promote effective utilization of healthcare services. Promotes/supports quality effectiveness of Healthcare Services.

Fundamental Components:
Directly supports Healthy Outcomes Department Leaders and Team Members providing Medicare Member outreach. Performs non-medical research, prints/mails letters and TPAs to Medicare Members, monitors centralized voicemail line, locates alternate phone numbers for Medicare Members, documents results/findings in spreadsheets/database, re-open cases, performs case count validation after scheduled caseloads, verifies accuracy of and distributes various reports to Team Leaders, reviews reports and communicates to Team Members applicable actions and performs various ancillary tasks as needed.
Maintains accurate and complete documentation of required information that meets risk management, regulatory, and accreditation requirements. Adheres to HIPAA policies and Protects the confidentiality of member information and adheres to company policies regarding confidentiality.

Additional Job Information:
Effectively multi-task in a fast-paced environment. Document clearly and accurately with attention to detail. Effective communication; written, verbal and telephonic. Strong organizational skills. Ability to be flexible and accept routine changes in workflows. Posses skills to work independently, and as part of a team. Computer literacy in order to navigate through internal/external computer systems, including Excel and Microsoft Word. Ability to effectively participate in a multi-disciplinary team including internal and external participants.
