Care Management Associate

Primary Location: Dallas, Texas
Additional Locations: TX-Dallas
Support comprehensive coordination of medical services including Care Team intake, screening and supporting the implementation of care plans to promote effective utilization of healthcare services. Promotes/supports quality effectiveness of Healthcare Services.

Fundamental Components:
The Care Management Associate (CMA) would:
  • Outreach members for intake into Case Management
  • Complete initial screenings to determine medical needs
  • Provide assistance for care coordination
  • Fundamental components and physical requirements include but are not limited to:
  • Promoting communication, both internal and external
  • Enhance effectiveness of the program
  • Protects the confidentiality of member information
  • Adheres to company policies; Sedentary work involving significant periods of sitting, talking, active listening in an office based cubical setting. The CMA would need to effectively participate on a team which includes internal and external participants and have proficiency with computers to navigate through computer systems.

2-4 years experience as a medical assistant, office assistant. Candidates should possess: Good organizational skills. Typing skills 50 wpm. Computer proficiency Business telephonic experience Customer Service skills 2-4 years experience as a medical assistant, office assistant. Must be able to talk and type at the same time.

The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience.
