Socially Necessary Services Liaison

The SNS (Socially Necessary Services Liaison) is responsible for providing services to improve relationships and social functioning, with the goal of preserving the individual's tenure in the community or the integrity of the family or social system. Socially necessary services (SNS) are interventions designed to maintain or establish safety, permanency and well-being for targeted populations.

Fundamental Components:
Fundamental Components/Job Description:
  • Monitor utilization of socially necessary services (SNS) and ensures member access to SNS
  • Collaborates and interfaces with Bureau for Children and Families (BCF) and all SNS providers to facilitate member access to care and positive health and social outcomes to maintain safety, permanency and well-being
  • Supports the administration of retrospective reviews of SNS providers
  • Supports members to avoid unnecessary disruption to their SNS, their environments, and their living situations
  • Proactively outreaches to members through routine ISP meetings, UM, member education, and reassessment to identify members at risk of SNS disruptions
  • Fosters and facilitates strong SNS provider relationships to support routine communication and integration of up to date, external, clinical, and community-based care plans within their Aetna service plan
  • Utilizes technology platforms that encourage SNS provider/health plan communication, such as Aetna's FamilyConnect
  • Disseminates user-friendly member and caregiver education tools by the member's assigned care coordinator, member advocates, reports gaps in service and how to access the 24-hour Health Information line (Nurse Line) and the Behavioral Health Crisis Services hotline and available community-based crisis support resources
  • Develops and implements work and project plans to achieve business goals
  • Solicits and evaluates internal and external customer feedback to enhance continuous quality improvement
  • May conduct group facilitation/presentations
  • Identifies issues that may impact a deliverable or project and addresses them by either resolving the issue or creating an alternative solution
  • Tracks progress and communicates work plan or project status on a regular basis to all impacted parties

Background Experience:
Knowledge and Skills:
  • Minimum of 3 years’ experience working with SNS providers is required
  • Candidate should be a resident of state
  • Proven communication skills (written, verbal and presentation)
Education and Certification Requirements:
  • The highest level of education desired for candidates in this position bachelor’s degree in Social Work, Counseling, Human Services or a relevant field is required.
