Case Manager RN Registered NursePrimary Location: Albany, New York

The Case Manager for the Healthcare Vertical team utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit design and/or acute health needs through communication and available resources to promote optimal, cost-effective outcomes.
This role is located in the Albany, NY office. This is a dynamic and passionate team, with much support! This position is in an office environment using a computer station and may require sitting for extended periods of time. - Candidate must possess computer and typing skills. - This is ideal for an applicant seeking a fast paced environment. Full Time positions with business hours, Monday through Friday 8am-5pm. To service west coast customers, nurses will be required to work 2-3 times a quarter until 9:00pm EST after training to meet federal and state regulated turn-around times. No weekends. Holiday coverage requirements.
Fundamental Components but not limited to the following:
* Understanding Clinical Impacts
* Providing Solutions to Constituent Needs
* Providing education to promote health engagement
* Working closely with members to implement an individual plan of care
5 years clinical practice experience
Disability case management experience preferred
The minimum level of education desired for candidates in this position is a Associate's degree or equivalent experience.

Nursing/Registered Nurse (RN) is required active and no restrictions
Nursing/Certified Case Manager (CCM) is desired
Functional - Nursing/Case Management/1-3 Years
Functional - Nursing/Discharge Planning/1-3 Years
